To order the "Tooth Fairy" letter, please complete the form below and click "Submit".  Cost of the letter is $12.50 each.  If you choose to pay with a check/money order, mail to the address at the bottom,  or pay with a credit card using PayPal.   When payment is received your child will receive their very special letter along with some special surprises from the Tooth Fairy herself, such as a toothbrushing chart, crayons, stickers, puzzle and coloring  sheets and more!
